Incident Reports

In the event of an accident or incident, students and staff should notify the Head of Department or supervisory staff (e.g. lecturer), as soon as possible after getting first aid treatment. The Head of Department or staff member to whom the accident or incident is reported must complete the Accident/Incident Report Form within 24 hours. Staff or students may also report the accident or incident directly using the link. The aim of accident reporting is to learn from the event and prevent reoccurrence. Reporting is also a legal requirement.

Note: An accident is an incident which has given rise to injury or ill health or fatality. An incident where no injury, ill health or fatality occurs may also be referred to as a ‘near miss, ‘near hit’, ‘close call’
